- Check rhythm – This step requires an AED.
When should the EMT initiate chest compressions on an infant?
If an infant has a pulse rate of less than 60/minute with poor perfusion, CPR must be initiated. Bradycardia in infants causes the cardiac output to be insufficient for adequate perfusion and cardiac arrest may be imminent.

Offline medical direction means medical direction that does not involve voice communication provided to EMTs providing direct patient care.
How long should EMS spend on scene?
The reports were retrospectively collected and analyzed. Our data suggest that the optimal 20-minute STI for OOHCA patients in an urban EMS system is between 41 and 60 minutes. Interestingly, the 10-minute interval within the 41-60 minute cohort that provided the highest rate of ROSC was between 41 and 50 minutes.
