Health365 is an online service provided in partnership with Fifth Avenue Family Practice. It gives you secure access to your own medical records and a wide range of other services that make it easy to manage your own healthcare.

How do I reset my health 365 account?

Please follow the below instructions: a) Tap on “Profile” on the menu bar of the Healthy 365 mobile app. b) Tap on the “Restore profile” button at the top of the page. c) Key in your NRIC and mobile number (as entered during registration) and tap on “Restore Now”.

Does Health 365 have an app?

The app seamlessly pairs with fitness tracking devices to help users log their daily steps count and amount of time spent on active exercises. Users can also scan QR codes via the app to earn Healthpoints when they purchase healthier meals, drinks and groceries from participating partners.

Why is my Healthy 365 app not working?

Here is a list of the compatible trackers or mobile applications that can be connected to the Healthy 365 app: Actxa® mobile app. Apple Health mobile app. Fitbit mobile app with any Fitbit tracker.

How do I pair Health 365?

On the ‘Home’ tab of the Healthy 365 mobile app, tap on ‘Pair your tracker or app’ and select ‘HPB Trackers’. If you are connecting via QR Code, you will need your HPB fitness tracker’s packaging. If you are connecting via Bluetooth, you will need to turn on the Bluetooth® on your smartphone (at least 1.5m radius).

How do I get HPB steps Tracker for free?

Go to ”Home” page, look for the “Book & Manage an appointment” section. Tap “Book & Manage”. Tap on “National Steps Challenge™ Season 6” and agree to the Terms & Conditions to join the challenge if you have not yet done so.

Is HPB tracker still available?

Season 6 fitness trackers are only available for collection from 11 Oct 2021. Make an appointment to collect a HPB fitness tracker on the Healthy 365 app….HPB fitness trackers are available for collection from 6 June 2022 onwards.

Where can I find my national health number?

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter.
  • call your GP surgery and ask them for your number.

Are old NHS Numbers still valid?

If you have an old medical card, it will have an old style NHS Number made up of both letters and numbers. This has now been replaced for all patients by a new NHS Number made up entirely of numbers. Having the old style NHS Number will not affect your care.
